AtStoney Creek Family Dental, it is our pleasure to welcome you to our family friendly dental office. The primary goal of our dental practice is to serve the Riverbend and surrounding communities in helping achieve their best smile in the areas of health, look and function. We are a family focused dental practice that places a priority on a relaxed and enjoyable patient experience and respectful, trusting relationships. YOUR FIRST VISIT

We are so excited to see you for your first dental appointment with us! Upon your first visit to our office, you will be greeted by one of our friendly front desk team members. They will be available to assist you should you need any help filling out your forms. Please arrive approximately 10 minutes before your appointment time to complete the necessary forms we require prior to treatment.It is important that you bring with you information concerning your past medical history, dental insurance information and any medications you may be taking. If you wish to complete these forms prior to your appointment, please see the attachments below.

FINANCIAL POLICY

We are happy to offer direct insurance billing to most insurance companies as a courtesy for our valued patients! Please keep in mind that your insurance policy is an agreement between you, your employer, and your insurance company and it is the responsibility of the policy holder to notify our office of any policy changes, that way we are able to assist you in maximizing your dental benefits.

APPOINTMENT POLICY

Broken appointments are a disappointment to every; it can interfere with treatment progress and can create scheduling issues for future treatment.We strive to accommodate the needs of our valued patients by providing the best possible dentistry, treatment options, and service available. We accomplish this by scheduling each patient in an especially reserved time for your individual treatment needs. When an appointment time is agreed upon we feel a commitment to the scheduled time and treatment has been made.We request two business days’ notice to change an appointment time. This allows the Stoney Creek Family Dental Team time to manage and respect our doctors and dental hygienists’ time accordingly. We understand, in rare circumstances, that emergencies occur and these appointments will be assessed individually.
